Tuberculosis still remains a serious world-wide problem. This applies chiefly to the developing countries of Africa or India. In our country and in western Europe, tuberculosis seemed to be under control until the spread of HIV infection began. With the development of tourism and arrival of immigrants, the possibility of contact with this disease has increased. In addition, the situation is complicated by the fact that, at early stages, the diagnosis of both TB spondylitis and pulmonary tuberculosis is difficult. When the spine is involved, angular kyphotisation of the affected region may result in severe pain and neurological complications. For this reason, early diagnosis is necessary and should be followed by appropriate treatment, i.e., radical management of the lesion on the spine and anti-TB drug therapy. To establish the diagnosis, plain X-rays, bone scan, CT examination and magnetic resonance imaging of the spine are used in addition to strandard biochemical tests. The diagnosis is confirmed by histological examination and DNA assessment by PCR.

We conducted a double-blind, randomized, placebo-controlled trial of cyproheptadine for treating sleep problems found in PTSD. The participants were male Vietnam veterans who had current combat-related PTSD according to the Clinician Administered PTSD Scale 5 ; and who also reported at least moderately severe nightmares on the Pittsburgh Sleep Quality Index 6 ; . The exclusion criteria included current substance abuse, use of a selective serotonin reuptake inhibitor, mania or hypomania, and any medical condition that contraindicated the use of cyproheptadine. After complete description of the study to subjects, written informed consent was obtained. Sixty-nine subjects were enrolled in this 2-week trial across two sites. Posttreatment data on the Clinician Administered PTSD Scale, the Pittsburgh Sleep Quality Index, and a nightmare questionnaire were available for 60 subjects. The drug and placebo groups did not differ at pretreatment on severity scores on the Clinician Administered PTSD Scale F 2.06, df 1, 56, p 0.16 ; , total scores on the Pittsburgh Sleep Quality Index F0.00, df 1, 56, p1.00 ; , or nightmare severity F 2.80, df 1, 56, p 0.10 ; . When adjusted for pretreatment scores by analysis of covariance, posttreatment scores on the Clinician Administered PTSD Scale F 0.06, df 1, 55, d 0.14, p 0.81 ; and scores for nightmare severity F 1.92, df 1, 55, d 0.37, p 0.17 ; were nonsignificantly higher worse ; in the treatment group than in the placebo group, and scores on the Pittsburgh Sleep Quality Index showed marginally poorer sleep in the treatment group than in the placebo group F 3.68, df 1, 55, d 0.58, p 0.06 ; . Cyprohfptadine serum levels determined by gas chromatography mass spectrometry ; were available at one site for 14 of 15 treated subjects. Partial correlation analysis, controlling for pretreatment scores, showed a marginally significant correlation of higher cyproheptadine levels with a worsening of Pittsburgh Sleep Quality Index scores r 0.47, p 0.051 ; but no significant correlation with scores on the Clinician Administered PTSD Scale r 0.21, p 0.25 ; or scores for nightmare severity r 0.24, p 0.22 ; in one-tailed tests ; . Contrary to expectation 1, 2 ; , cyproheptadine does not appear to be an effective treatment for sleep problems or combat-related PTSD and may even exacerbate sleep disturbance. Although the study group was relatively small, low power is an unlikely explanation for our nonsignificant findings because of the trend for poorer sleep in the treatment group and the likely correlation of cyproheptadine levels with worsening of sleep. Our results reinforce the need for skepticism about open-label or anecdotal findings and for careful scientific trials to replicate uncontrolled studies and diclofenac.

The treatment of Cushing's disease is a challenge for both endocrinologists and neurosurgeons. Pituitary adenomectomy is the therapy of choice, achieving a 70 80% cure rate, but disease may recur in nearly 13 25% of the cases, and recurrence progressively increases over the years 7 9 ; . Although several neurotransmitters and neuropeptides influence ACTH secretion, currently available medical approaches offer little therapeutic success: the administration of neuroactive drugs acting at the hypothalamic pituitary level such as bromocriptine, cyproheptadine, cabergoline and valproic acid ; seldom shows clinical efficacy and, above all, the long-term responsivity is low 12 ; . Recently, Heaney and coworkers demonstrated that rosiglitazone inhibits ACTH and corticosterone secretion in treated mice, and suggested that thiazoledinediones may be effective as therapy for Cushing's disease 6, 13 ; . In the present study, rosiglitazone was chronically administered in a group of patients with Cushing's disease, in order to evaluate its capability to reduce ACTH and cortisol secretion. Indeed, about 40% of them showed a reduction of UFC after 1 2 months of treatment, and an almost apparent normalization of UFC occurred in six cases. It is worth noting that in responsive patients rosiglitazone positively influenced cortisol secretion already after a short period of time. Very recently, the effects of rosiglitazone administration in two patients with Cushing's disease have been briefly reported: the treatment for 3 12 weeks ; was able to reduce UFC excretion by 55% in one patient and to normalize it in the other 10 ; . Our data show a more evident influence of rosiglitazone on cortisol secretion than on ACTH secretion, and this is in line with several observations. In fact, it was previously demonstrated that PPAR-g agonists inhibit ovarian steroidogenesis 14 in addition, a direct effect of rosiglitazone on the steroidogenic enzymes P450c17 and 3b-hydroxysteroid dehydrogenase was proven by in vitro studies 15 ; . These androgen-lowering effects may further explain the documented clinical efficacy of thiazolidinediones in ameliorating hyperandrogenism in women with polycystic ovarian syndrome 14, 16 ; . Moreover, the potential role of thiazolidinediones in ameliorating Cushing's disease is supported by the observations that these drugs antagonize the action of glucocorticoids on target organs, with respect to multiple metabolic effects 17 ; .
