The assay was developed using EDN purified from pooled urine from healthy volunteers, to which polyclonal and monoclonal antibodies were raised. Where this is not possible, enquiry should always be made about partner sexual health and satisfaction. It may be essential to address a partner sexual function problem in order to effectively treat ED [5]. artners with aversion to sex, low desire [53], arousal problems and sexual pain disorders may not allow the man to have sex with them [54]. Some men may present this to their doctor as an erection problem, either from a genuine belief that this is the cause of their dissatisfaction and, almost certainly incorrectly, that improving their erection will resolve their partner's problem. An improved understanding of the similarities and differences in sexual interest and response in men and women may be beneficial. The clinician should be able to provide simple behavioural advice regarding foreplay, sexual activity and on the integration of medication into the couple's sexual behaviour. Formal cognitive-behavioural interventions should be provided by appropriately trained and experienced therapists. They may be of some benefit in all men but are probably best used in men with a predominantly psychogenic component in ED. Such interventions are less likely to be beneficial in men with complete ED of predominantly organic aetiology. Concurrent use of medication, such as DE5 inhibitors, is not precluded in men engaged in cognitive-behavioural therapy, and a combined approach may be more effective than using these interventions individually or consecutively [55]. C.6 Treatment of ED after radical prostatectomy A very high proportion of men develop acute ED after radical prostatectomy. This is thought to be predominantly due to neural damage incurred during surgery. The cavernous nerves that modulate penile vascular smooth muscle tone are found in the neurovascular bundles adjacent to the prostate gland and, even if not transected, are susceptible to trauma during radical prostate surgery. Where a nervesparing procedure has been performed, there is often a gradual improvement in neural function, but this improvement may take up to two years. Consequently, men with probable neurogenic ED following radical prostatectomy have, initially at least, healthy cavernosal vascular smooth muscle and structural integrity. rovided that this is maintained, they should recover erectile function concurrently with their cavernous nerve function. However, in men who develop persistent ED after surgery, cavernosal smooth muscle is gradually replaced by collagenous tissue [56]. Increased drug efflux, and altered or increased C-14a demethylase activity.2 Keetoconazole Ketocnazole can be used to treat both superficial and systemic fungal infections. It can be used to treat chronic mucocutaneous candidiasis, coccidiomycosis and other infections in non-immunosuppressed patients, as well as prophylaxis of mycoses in immunocompromised patients. It is taken orally at a dose of 200400mg per day.Hepatitis is a rare complication, affecting around 1 in 15, 000 patients, and it has been implicated in fatal hepatotoxic reactions. Ketoconzzole commonly causes anorexia, nausea and vomiting, which can be relieved by taking the drug with food. It is metabolised by the liver, and periodic liver function tests should be undertaken in patients taking more than 14 days of treatment. The triazoles fluconazole and itraconazole can also be given once daily, due to their long half lives 2030 hours ; but they also exhibit fewer side effects than ketoconazole, and are well absorbed after oral administration. Fluconazole Fluconazole can be given intravenously or orally, and has an oral bioavailability of around 80 per cent. It distributes into a number of body fluids and also has good penetration into the brain and cerebrospinal fluid.4 Fluconazole can be used to treat superficial and systemic candida infections, cryptococcal meningitis and coccidioidal meningitis. For superficial or mucosal infections, fluconazole is usually given in a dose of 50100mg daily.A single dose of 150mg can be used for vulvovaginal candidiasis. For invasive candidal infections, a loading dose of 400mg is usually given, followed by 200mg once daily. A dose of 400mg per day can be used for severe infections. Fluconazole can also be used for prophylaxis of fungal infections in neutropenic patients. Furthermore, several lines of evidence suggest that the Npr2p protein is also involved in the regulation of the nitrogen permease Dur3p at the post-transcriptional level Rousselet et al., 1995 ; . Therefore, we initially hypothesized that NPR2 might mediate cisplatin- and doxorubicin-induced cell kill through a DUR3-dependent pathway.
